Modivcare Non-Emergency Medical Transportation Services

NJ Family Care members need help getting to medical appointments.

To address this concern, Medicaid covers “non-emergency medical transportation (NEMT)” rides for eligible individuals to obtain medically necessary, covered services.

This coverage is called “non-emergency medical transportation,” because it does not involve a medical emergency.

Modivcare, the state’s medical transportation broker, provides non-emergency medical transportation to NJ family Care clients living anywhere in the state.

How to Schedule Transportation:

Reservations can be made Monday- Friday 8:00 to 5:00 p.m. local time. Please call at least 2 business days in advance to make a reservation, but not more than 30 days before the appointment.

Please have the following available when making your reservation:

  • Your Member ID number
  • Name & address of medical providers
  • Appointment day and time

Please note, the mileage limit for NEMT transportation is 35 miles one way. Any trip over thirty-five (35) miles will be screened to obtain Closer Provider Certification (CPC) from the member insurance plan.
